1️⃣ Special Report

What it means:

  • A special report is needed when a service is:
    • Rarely performed
    • Unusual
    • Variable
    • Not clearly described by an existing CPT code

What the report must include:

  • Nature of the service
  • Extent of the service
  • Need/medical necessity
  • Time, effort, and equipment used

📌 CPC Tip:
If documentation is insufficient or service is unusual → Special report required


2️⃣ Time (Very Important for CPC)

General rule:

  • Time is face-to-face time with the patient (unless stated otherwise)
  • Time rules apply unless guidelines say differently

⏱ Mid-Point Rule

  • A unit of time is counted only after the midpoint is passed
  • Example:
    • 30–60 minutes → midpoint = 45 minutes
    • Must reach 45 minutes to count that unit

⏱ E/M Codes

  • Use total time on date of encounter
  • Midpoint rule does NOT apply for E/M

⏱ Concurrent Services

  • If two time-based services happen together →
    Do NOT count time twice

⏱ Continuous Services

  • If service crosses midnight →
    Report total continuous time
  • Service does not reset at midnight

  • Never reported alone

  • Codes for pending FDA approval (vaccines)

📹 Telemedicine Symbols

  • Used to identify:
    • Audio-video telemedicine
    • Audio-only telemedicine
  • Refer to Appendix T

🔁 ⧈ (Resequenced codes)

  • Code is out of numeric order
  • Used to keep related services together
  • Still a valid CPT code

📌 CPC Trick:
Resequenced ≠ invalid code


4️⃣ Proprietary Laboratory Analyses (PLA)

What are PLA codes?

  • Lab tests developed by:
    • Single lab (sole source) OR
    • Multiple labs with FDA approval

Key points:

  • Listed at end of Pathology & Lab section
  • Identified by ⧈ symbol
  • Do NOT qualify as Category I codes
  • Name may be proprietary (company name)

📌 Exam clue:
PLA ≠ Category I

  • Helps you search by:
    • Procedure name
    • Anatomy
    • Common terms / synonyms

📌 Rule:
Always verify the code in the Tabular List
