1️⃣ Format of the Terminology (VERY IMPORTANT)

CPT codes are written in a short-cut style to save space.

👉 What this means:

  • Sometimes the full procedure name is NOT repeated
  • You must read the previous code description

📌 Example (Exam favorite):

  • 25100 – Arthrotomy, wrist joint, with biopsy
  • 25105 – with synovectomy

👉 Code 25105 actually means:
Arthrotomy, wrist joint, with synovectomy

🧠 Exam rule:

If words are missing → borrow from the code above


2️⃣ Indented codes = continuation

  • Indented codes depend on the main code
  • They do NOT stand alone

🧠 CPC trick:
Indented code = read above description


3️⃣ CPT Nomenclature can be updated

  • CPT codes change because medicine changes
  • New procedures → new codes
  • Old procedures → revised or deleted codes

🧠 For exam:
CPT is NOT static, it is updated every year.


4️⃣ Who updates CPT codes?

  • AMA (American Medical Association)
  • Doctors
  • Specialty societies
  • Health professionals

👉 Coders DO NOT create codes, they only use them.


5️⃣ General CPT Code Criteria (EXAM GOLD ⭐)

A CPT code must:

✅ Be clear & unique

  • Describes one specific procedure
  • Not confusing with another code

✅ Follow CPT rules

  • Must match guidelines & instructions

❌ Not fragment an existing code

  • Don’t break one service into many codes
  • Don’t duplicate existing codes

🧠 Keyword:
No fragmentation = No unbundling


6️⃣ Combination procedures

  • If procedures are usually done together
  • CPT may create ONE combined code

❌ Don’t report parts separately
unless CPT allows


7️⃣ Extraordinary circumstances NOT separately coded

  • CPT codes describe normal procedures
  • Complications or unusual difficulty
    → usually NOT coded separately

8️⃣ Category I CPT Code Requirements (VERY IMPORTANT)

For a procedure to get a Category I code, it must:

✅ FDA approved

  • Devices & drugs must have approval (if required)

✅ Commonly performed

  • Done by many physicians
  • Across many locations

✅ Frequently used

  • Common condition → high volume
  • Rare condition → low volume

✅ Accepted medical practice

  • Standard care
  • Not experimental

✅ Proven in literature

  • Clinical evidence supports it
