1️⃣ Special Report

  • A special report is used when:
    • Service is rare / unusual
    • Extra time, effort, equipment, or skill is required
  • Documentation must explain:
    • Nature of service
    • Extent
    • Why it was necessary
      📌 Exam tip: Special report = extra explanation needed

2️⃣ Time (Very Important for CPC 🔥)

  • Many CPT codes are time-based
  • Mid-point rule applies unless stated otherwise
    • Example:
      • 30–60 min code → must cross 31 minutes
      • 60–90 min code → must cross 91 minutes
  • Face-to-face time with patient counts
  • Phrases like “interpretation and report”:
    • Time is included unless CPT says otherwise
  • E/M codes:
    • Use total time, NOT midpoint rule
  • Concurrent services:
    • Don’t count overlapping time twice
  • Continuous services across midnight:
    • Do NOT reset clock
    • Report total time
  • Example:
    • IV hydration from 11 PM–2 AM
    • Report once, total time

3️⃣ Code Symbols (Highly Tested 📝)

Symbols next to CPT codes mean important things:

  • ▲ (Triangle) – New or revised text
  • ● (Bullet) – New code
  • + (Plus) – Add-on code (never billed alone)
  • Ø (Circle with slash) – Modifier 51 NOT allowed
  • # (Hash) – Telemedicine / special reporting
  • ✱ (Star) – Proprietary Lab Analysis (PLA)
  • ↔ / resequenced codes – Code placed out of number order

  • Identified with ✱ symbol
  • Used for specific lab tests
  • May be:
    • Single-source lab
    • FDA-approved
  • Listed at end of Pathology & Lab section
  • PLA codes:
    • Do NOT meet Category I criteria
