📘 CPT Code Sections & Ranges (Must Remember)

Know the sequence + logic, not just numbers:

  • Evaluation & Management (E/M): 99202–99499
  • Anesthesiology: 00100–01999, 99100–99140
  • Surgery: 10004–69990
  • Radiology: 70010–79999
  • Pathology & Laboratory: 80047–89398, 0001U–0599U
  • Medicine (except anesthesia): 90281–99199, 99500–99607

  • Each page shows first & last code numbers at the top margin
  • Continuous pagination at bottom
  • Symbols explained on same page

🧠 CORE RULES for Using CPT Codes (Very Important for CPC)

✅ Code Selection Rules

  • Always choose the code that MOST ACCURATELY describes the service
  • ❌ Never choose a code that “approximately” matches
  • If no specific code exists → use unlisted procedure code
  • Unlisted codes must be clearly documented

✅ Read the FULL Descriptor

  • Read entire code description
  • Include parenthetical notes and text in brackets
  • Don’t skip small words — CPC loves trick questions

✅ Surgery vs Medicine

  • Placement of a code in Surgery or Medicine does NOT decide reimbursement
  • New techniques (endoscopic, minimally invasive) may blur the lines
  • Location in book may be historical, not functional

👩‍⚕️ Who Can Report CPT Codes?

  • Physician or Qualified Health Care Professional
  • Clinical staff can assist but cannot independently report
  • Services may be restricted by law, regulation, or facility policy

🚫 Bundling & Unbundling Rules (HIGH-YIELD)

  • Some codes include multiple components (bundled)
  • ❌ Do NOT unbundle bundled services
  • Parenthetical notes often tell you:
    • “Do not report with…”
    • “Included in…”

Example:

  • Tonsillectomy + Adenoidectomy in child <12
    One combined code, not separate codes

🧩 Component Procedures

  • Components of a main procedure are NOT separately reportable
  • Steps to reach or close the surgical site are included
  • Incision & closure usually not coded separately

📌 Exception:

  • Complex repair may be reported separately if documented
